Acute Liver Injury and COVID 19 Disease Severity in a Tertiary Private Hospital in the Philippines

Authors: Billy Joseph David, Hanna Clementine Tan, Armin Masbang, Jose Guillain Cataluña

DOI: 10.65564/pjim.357b50d629   |   Published: September 2024

Coronavirus disease 2019 (COVID-19) has been associated with acute liver injury presenting as increased liver enzymes, specifically alanine aminotransferase (ALT) and aspartate aminotransferase (AST). There is limited data in the prevalence of liver injury in COVID-19. We aim to determine the prevalence of acute liver injury among COVID-19 patients admitted in a tertiary hospital in the Philippines.

Association Between Body Mass Index and Cognitive Impairment in Elderly Subjects with Type 2 Diabetes Mellitus: A Cross-Sectional Study

Authors: Maria Guia Estrella A. Dela Cruz, Michelle Co, Carter Rabo

DOI: 10.65564/pjim.14760c4645   |   Published: September 2024

Chronic illnesses such as Type 2 diabetes mellitus (T2DM) and obesity have been implicated as risk factors in the development of cognitive impairment (CI), but despite this, definite association between the two conditions in increasing cognitive impairment risk is not well defined. This study aims to examine the association between body mass index (BMI) and cognitive impairment (CI) in elderly patients with Type 2 diabetes mellitus.

DOI: 10.65564/pjim.101f475110   |   Published: September 2024

Macrophage Activation Syndrome (MAS) is a rare but life threatening pro-inflammatory complication of multiple autoimmune diseases leading to cytokine storm. We report a case of MAS as a presenting manifestation of Systemic Lupus Erythematosus. A 32-year-old female, newly diagnosed with Systemic Lupus Erythematosus (SLE), presents with a 3-month history of fever and joint pains, which began a few days after receiving her first dose of a viral vector COVID-19 vaccine. She later developed facial edema, and her fever became persistent and unremitting.

Transverse Myelitis Unmasking Multiple Sclerosis After mRNA COVID-19 Vaccine: A Case Report

Authors: Javish R. Jadwani, Eloise D. Guadaña, Cristina A. Dorado, Alexander D. Abe, Jay S. Fonte

DOI: 10.65564/pjim.31aa5e8166   |   Published: September 2024

Demyelinating central nervous system (CNS) disorders such as transverse myelitis (TM) and multiple sclerosis (MS) have been reported with mRNA Covid-19 vaccine. Some cases were relapses of a pre-existing condition but de novo and initial presentation of MS after BNT162b2 COVID-19 mRNA vaccine has very rarely been documented. We report a 72-year-old female, right-handed, Filipina, with a one-month history of bilateral lower extremity weakness which occurred 7 days after she received her first booster dose of BNT162b2 mRNA vaccine.

A Rare Case of Coexistence: Papillary Thyroid Carcinoma Dedifferentiated to Squamous Cell Carcinoma: A Case Report

Authors: Mariel C. Enverga, Thelma D. Crisostomo

DOI: 10.65564/pjim.16e0b1a118   |   Published: September 2024

Squamous cell carcinoma (SCC) of the thyroid is a rare condition comprising less than 1% of all thyroid malignancies. Thyroid SCC is a variant of undifferentiated thyroid carcinoma and is also associated with the tall-cell variant of papillary thyroid carcinoma (PTC) and spindle cell carcinoma. Given its aggressive clinical course, early recognition of the disease is essential to management. We report a rare case of a 67-year-old, Filipino male initially presenting with an anterior neck mass over a period of 11 months.

Gastric Outlet Obstruction arising from Adhesions Secondary to Chronic Calculous Cholecystitis with Cholecystoduodenal Fistula Formation in an Immunocompetent Male: A Case Report

Authors: Christmae Maxine P. Solon, Janrei Jumangit, Daniel Benjamin Diaz, Karen Batoctoy

DOI: 10.65564/pjim.4c4e770685   |   Published: September 2024

Gastric outlet obstruction (GOO) results from intrinsic and extrinsic obstruction of the pyloric channel or the duodenum. Here we present a rare case of GOO attributed to dense adhesions between the gallbladder and duodenum secondary to chronic cholecystitis with choledococystoduodenal fistula formation. Previous reports identified elderly females with comorbidities as a predisposing factor; however, our patient was an immunocompetent adult male.
