Health Seeking Behavior of Patients with Ischemic Heart Disease during the COVID-19 Pandemic in the Outpatient Clinics of a Tertiary Hospital

Authors: Marrjhourie Rose B. Bringas, Karla Rhea Rillera-Posadas, Sofia Rellema-Black

DOI: 10.65564/pjim.bbcceb312c   |   Published: December 2023

Ischemic Heart Disease is a chronic, progressive, and dynamic disease. It remains to be the number one cause of mortality globally and in the Philippines. Patients with IHD belong to the vulnerable group both before and during the COVID 19 pandemic. Currently, there have been changes in the healthcare system leading to decreased delivery of services such as outpatient care and development of medical care avoidance affecting patients’ clinical outcomes. This study aimed to determine if there are any changes in the health seeking behavior of these patients during the pandemic in a single tertiary hospital.

Outcomes and Management of ST-Elevation Myocardial Infarction Patients Before and During the COVID-19 Pandemic: A Retrospective Cohort Study

Authors: Miguel Angelo D. Ang Co, Rhandy P. Panganiban

DOI: 10.65564/pjim.eb2cf98342   |   Published: December 2023

Ischemic heart disease is the leading cause of mortality and morbidity worldwide. The COVID-19 pandemic changed healthcare-seeking behavior and healthcare delivery. This is a single-center, retrospective cohort study using a non-probability sampling of adult patients at the Philippine Heart Center who were diagnosed with ACS-STEMI.

Development of a Risk Prediction Score for Acute Kidney Injury in Critically-ill Septic Filipino Patients Admitted in Perpetual Succour Hospital

Authors: Jane Lou E. Gargaritano, Joel John C. Mejos, Jewelyn M. Torrevillas, Giovanni A. Vista, Grecia Darunday, Eratosthenes S. Polito

DOI: 10.65564/pjim.4053e4eb14   |   Published: December 2023

Acute kidney injury (AKI) is a lethal complication of critical illness characterized by the rapid loss of the kidney's excretory function encountered in 50% of intensive care unit (ICU) admissions. Its impact on the outcome of critically ill patients makes AKI a significant cause of morbidity and mortality. The objective of this study was to develop and validate an acute kidney injury risk prediction score based on routinely available variables and common laboratories of admitted critically-ill septic Filipino patients.

Relationship of Serum Levels of Bone Specific Alkali Phosphatase (bALP) and Alkali Phosphatase (ALP) with Vascular Calcification in Chronic Kidney Disease Patient on Chronic Hemodialysis

Authors: Bayu Rusfandi Nasution, Harun Rasyid Lubis

DOI: 10.65564/pjim.8ea166015c   |   Published: December 2023

Vascular calcification is an important non-conventional for cardiovascular disease (CVD) in chronic kidney disease (CKD) patients with chronic hemodialysis (HD). Abdominal aortic calcification (AAC) is reported as an independent predictor for cardiovascular morbidity and mortality. Bone-specific Alkali Phosphatase (bALP) and Alkali Phosphatase (ALP) enzymes are produced and released when changes or disorders of bone and mineral metabolism occur. Given biomarker studies such as bALP and ALP which are more often associated with patient mortality, more research will be needed to assess whether these bALP and ALP biomarkers have a linear distribution of relationships with vascular calcification. This study aimed to evaluate the serum biomarker to predict calcification and further can be one of diagnosis modality of calcification in hemodialysis patients.

Maternal and Neonatal Outcomes Among Women with Gestational Diabetes Mellitus Treated with Metformin in a Tertiary Hospital: A Retrospective Cohort Study

Authors: Francis Xavier Fernandez Mislang, Rebecca Lim Alba

DOI: 10.65564/pjim.fb65388702   |   Published: December 2023

Gestational diabetes mellitus (GDM) is a common disorder associated with both maternal and fetal complications. Treatment for GDM requires lifestyle modification, as well as insulin and oral anti-diabetes medications to prevent unwarranted fetal and maternal outcomes. The objective of this study was to determine the feto-maternal outcomes of GDM patients treated with either metformin-monotherapy, insulin-monotherapy, and with insulin plus metformin (combination) therapy in a private tertiary hospital in Metro Manila.

Clinical Profiles and In-hospital Short-term Outcomes of Suspect, Probable, and Confirmed Adult COVID-19 Patients at the Philippine Heart Center: A Descriptive Study

Authors: Ana Katrina C. Longos, Maria Teresa B. Abola, Irene S. Bandong, Ma. Encarnita C. Blanco-Limpin, Alexander A. Tuazon, Lorraine Grace Almelor-Sembrana

DOI: 10.65564/pjim.ff2140f549   |   Published: December 2023

COVID-19 emerged as a new disease during the early period of 2020. Given that our institution is a cardiac specialty center and our patients have numerous co-morbidities compared to the general population, we wanted to determine the clinical profiles and in-hospital short-term outcomes of suspect, probable and confirmed adult COVID-19 patients seen at our institution.

The Dawn of Handheld Echocardiogram in the Philippines: A Narrative Review

Authors: Rainier John Buensalida, Jude Erric Cinco, Matilde Melanie Cheng-Co, Lea Farrah Abella

DOI: 10.65564/pjim.1e61f106a4   |   Published: December 2023

Handheld Echocardiogram (HHE) is a small ultrasound machine used to acquire echocardiographic images and data. Today, ease on end-user and patient-friendliness paved the way to the extensive reach in remote areas, low-resource regions, and emergency settings. This state-of-the-art review aims to provide data on HHE utilization in the Philippine setting.

Co-occurrence of Cerebral Stroke and ST Elevation Myocardial Infarction associated with Protein S Deficiency: A Case Report

Authors: Kristina Marie Michelle O. Rivera, Aaron Christopher P. Novales, Bryan Rene F. Toledano, Kristy B. Garganera, Frederick S. Gabriel, Gina V. Alemany

DOI: 10.65564/pjim.44c6a795fc   |   Published: December 2023

In the coagulation cascade, Protein S serves as a co-factor of protein C that indirectly prevents thrombosis by decreasing thrombin generation. Deficiency of Protein S predisposes to a hypercoagulable state that rarely leads to arterial thrombosis. We report a first local case of acute myocardial infarction occurring within days after a cerebral stroke or during the same admission associated with Protein S deficiency.

Diagnostic Dilemma: Postpartum Pneumonia Secondary to Community-Acquired Pneumonia versus an RT-PCR Negative COVID-19 Infection

Authors: Antonella Faye G. Go, Bayani Pocholo T. Maglinte, Jeremyjones F. Robles

Post-acute SARS-CoV-2 infection occurs beyond the two weeks of acute infection period. This hyperinflammatory state following the initial phase of viral replication and immune responses differs from the inflammatory state found in the acute infection. It affects multi-organ systems specifically pulmonary, cardiovascular, gastrointestinal, dermatologic, and musculoskeletal. The explanation behind this phenomenon is hypothesized to be brought about by a dysregulated host response against the COVID-10 infection. This is a case of a 37-year-old postpartum woman who presented with progressive dyspnea three weeks after delivery via a repeat cesarean section.
