A physician’s folly? Issuing a medical certificate “not for medicolegal purpose”

Authors: Rodel V. Capule

DOI: 10.65564/pjim.10aba2ea1e   |   Published: March 2021

The widespread use of medical certificates marked “Not for medicolegal purpose” has become common practice among some physicians, despite the legal weight such documents inherently carry. Disclaimers cannot prevent patients from using the certificate for legal purposes, nor can they exempt the issuing physician from potential court involvement, underscoring the need for careful, justified use.

DOI: 10.65564/pjim.d535fee33f   |   Published: March 2021

  1. The Philippine National Deployment and Vaccination Plan for COVID-19 espouses a whole-of-society approach for the successful vaccination for all Filipinos.
  2. Patients with Autoimmune Inflammatory Rheumatic Diseases (AIIRD) may benefit from COVID-19 vaccination.
  3. Patients with AIIRD must be informed of the possibility of a reduced response to vaccination and the importance of adhering to protocols to avoid infection.
  4. Post-vaccination monitoring is important.
  5. Patients with AIIRD who have questions regarding COVID-19 vaccination should consult their rheumatologist to arrive at a shared decision.

Fibroblast growth factor-23 (FGF23) is a circulating regulator of phosphate and vitamin D metabolism and has been implicated as a putative pathogenic factor in cardiovascular disease. The objectives of this study were: to compare serum FGF23 levels between systemic sclerosis (SSc) patients and healthy controls and to investigate possible associations between FGF23 and serum lipid profile in SSc patients.

Acute Kidney Injury in COVID-19 is Associated with Mortality: A Meta-Analysis

Authors: Ni Made Putri Lastiana, Marianto, Dian Daniella

DOI: 10.65564/pjim.64a160689d   |   Published: March 2021

By March 2020, The World Health Organization (WHO) has declared Coronavirus disease-19 (COVID-19) as a global pandemic. Further investigations found that COVID-19 may lead to acute kidney injury (AKI). Some studies have been done, but the incidence and outcome of AKI in COVID-19 are variable between studies. Moreover, given the high number of COVID-19 cases in our country, we aimed to perform a systematic review and meta-analysis regarding the detailed outcome of AKI in COVID-19 patients as reported in the available literature.

Spleen Factor: The Spleen’s Role as a Respiratory Organ

Authors: Rafael Lorenzo G. Valenzuela, Manuel C. Jorge, Mark Anthony S. Sandoval

DOI: 10.65564/pjim.1e778bb115   |   Published: March 2021

The Bajaus’ ability to free dive for prolonged periods is attributed to their exceptional physiologic response and large spleens. The spleen has been traditionally viewed as a hematologic and immunologic organ. Unconventionally, this review explores the spleen’s role as a respiratory organ and how apnea training can enhance the spleen’s form and function.

Secondary Renal Amyloidosis in Rheumatoid Arthritis Patient

Authors: Lilik Sukesi, Guntur Darmawan, Stefanie Yuliana Usman, Laniyati Hamijoyo, Ria Bandiara

DOI: 10.65564/pjim.15ac1ed4f2   |   Published: March 2021

Rheumatoid arthritis (RA) is one of systemic chronic progressive inflammatory disorders based on immunological disharmonies. Poorly controlled systemic inflammation in RA often leads to renal diseases such as secondary amyloidosis. A 30-year-old man complained of swelling and tenderness of multiple joints gradually worsened the past 7 years. His laboratory examination showed anemia, positive rheumatoid factor (RF) and anti-citrullinated protein antibody (ACPA).

Complete Excision of a Rare Case of Subependymal Giant Cell Astrocytoma (SEGA) in Tuberous Sclerosis Complex

Authors: Norman D. Pagar, Maria Anna De Guzman, Criscely L. Go

DOI: 10.65564/pjim.eb24959673   |   Published: March 2021

Subependymal giant cell astrocytoma is a rare tumor that occurs in the walls of the lateral ventricles, foramen of Monro, and less frequently, in the third ventricle. It is one of the intracranial lesions found in tuberous sclerosis complex (TSC) ─ a rare multisystem genetic disease. We present a rare case of an adult Filipino with cutaneous signs of TSC, who initially presented with signs of increased intracranial pressure.

Timely and Conservative Anticoagulation in Septic Cavernous Sinus Thrombosis: A Case Report

Authors: Kevin Paul DA. Enriquez, Muktader Kalbi, Salip Nastra Jumaani

DOI: 10.65564/pjim.18c0e7f3ac   |   Published: March 2021

Cavernous Sinus Thrombosis (CST) is a rare and life-threatening condition with antibiotics as the mainstay of therapy for those due to infection. While controversy exists, recent retrospective reviews using anticoagulation reveal potential mortality reduction with a low risk of adverse events such as intracranial hemorrhage (ICH). The optimal timing and duration of treatment are unknown. We report a 32-year-old female who presented with fever, headache, complete bilateral ophthalmoplegia, cellulitis, and a cranial MRV diagnostic of CST.
